Molina Healthcare is asking you to nominate individuals you know that are exceptional community volunteers and unsung heros for their Molina Healthcare Community Champions Award. This award was created in 2006 and honors “individuals that inspire others through selflessness and contributions that positively affect community members.”

There are six categories within which you can nominate individuals. They are
Volunteers    
Support Groups and Service Clubs 
Faith-Based Organizations    
Health Care Media 
Advocacy and Civic Leaders    
Education 

Winners will be honored at a dinner and awards ceremony on October 25, 2012. Winners will also receive a $1000 grant to “pay it forward” to an organization of their choice. You can find more information and a nomination form here.
